Wellness Connexion

We offer all members a free, confidential online Personal Wellness Assessment (developed by Vanderbilt University School of Medicine). A report at the end of the assessment will inform you of your current state of health in relation to others in your age and gender group. The summary will also give recommendations, which you may choose to follow-up on your own, or consult with a wellness coach provided by Baptist Health who will be on-site at the JCA to educate and motivate you to adopt lifestyle changes and achieve your goals. When appropriate, they may help members seeking healthcare and medical providers, but they are not a substitute for a personal physician.

What is Wellness Connexion?

The mission of JCA/Baptist Health Wellness Connexion is to improve the health and well-being of our members and community by assessing risks, encouraging healthy behaviors and complementing care.

How does it work?

Click on the “Wellness Assessment” button below. Take a confidential, online Personal Wellness Assessment developed by Vanderbilt School of Medicine. At the end of the screening, you can schedule an appointment with a Wellness Coach.

What will happen at a coaching session?

The Wellness Coach will review your wellness assessment report to help determine areas of health improvement and set goals to live a healthy lifestyle. She will do screenings for blood pressure, body composition, cholesterol, glucose and triglycerides. The coaches are able to help connect you with a primary care physician in your area if you need one.

What is the next step?

It’s up to you! Many members follow up on the status of their goals by touching base biweekly, monthly or as needed for support.

What is the cost?

As a valued JCA member, it is FREE for you!

What are the coaches' qualifications?

Cheryl Meyer, BSN, RN, CHC – Registered Nurse and Certified Health Coach. Cheryl has worked as a Registered Nurse, health services manager and health coach in a variety of clinical and institutional settings, as well as serving 20 years with the U.S. Navy in a medical capacity.

Who sees my information?

All information is completely confidential; the JCA has no access to members’ information.

Are there any other perks besides the coaching and biometric screenings?

  • assessment = t-shirt
  • biometrics = water bottle
