How do I begin a Wellness Assessment?
Click here to begin your Wellness Assessment. All JCA members receive a free and confidential online Personal Wellness Assessment which was developed by Vanderbilt University School of Medicine. At the end of the assessment you will receive a report regarding your current health in relation to others in your age and gender group. The summary also gives recommendations, which you may choose to follow up on your own, or consult with a wellness coach provided by Baptist Health who will be on-site at the JCA to educate and motivate you to adopt lifestyle changes to achieve your goals. When appropriate, Wellness Coaches may help members seeking healthcare and medical providers but they are not a substitute for a personal physician.
What will happen during a coaching session?
A Wellness Coach will review your assessment report to determine areas of health improvement while setting goals toward a healthier lifestyle. The Wellness Coach will screen your blood pressure, body composition, cholesterol, glucose and triglycerides. The coaches are able to help connect you with a primary care physician in your area if you need one.
What is the cost?
